Advanced Care Planning - Information Session FREE

This free information session brought to you by our Aging in Place Committee Co-op Connections project.

Serious illness, injury, and disability can happen at any age. Learn how to plan ahead to protect your rights and wishes. 

Join us and learn about the umbrella of advance care documents: representation agreements; advance directives; enduring powers of attorney; understanding hospital and residential care medical orders of intervention; no CPR orders; and medical assistance in dying. 

All BC health authorities have now mandated that ALL patients be consulted about the level of care they would want in an acute health situation. It is important to know this information before you get to hospital.

Facilitator Connie Jorsvik was an RN for 25 years, is a professional healthcare navigator and holds her certificate in Advance Care Planning. She is a local, provincial and national speaker on Advance Care Planning.
