An election ballot only includes the co-op name and the type and date of the meeting. The ballot is otherwise blank, leaving space for members to write in the names of the candidates they are voting for.
Even if nominations for the position of director have been submitted prior to the AGM, the names of the nominees should never be placed on the ballots in advance of the meeting. Pre-filled ballots can give advance nominees an unfair advantage by having their names appear first on the ballot.
